What is the Troponin-I Test?
The Troponin-I test measures the level of troponin I — a protein released into the bloodstream when heart muscle cells are damaged. Under normal circumstances, troponin is found only inside heart muscle cells and is undetectable in the blood. When the heart is injured — most commonly during a heart attack — troponin leaks into the bloodstream, making it the most specific and sensitive blood marker for heart muscle damage available today.
Why is the Troponin-I Test Done?
- To diagnose a heart attack (myocardial infarction) — the primary use of this test in emergency settings
- To risk-stratify patients with chest pain — a rising troponin confirms heart muscle injury
- To detect myocarditis — inflammation of the heart muscle caused by viral infection
- To evaluate heart damage in patients with severe infections, pulmonary embolism, or kidney failure
- To monitor cardiac complications after major surgery or chemotherapy

How to Prepare for the Troponin-I Test
- No fasting or special preparation is required
- This test is usually ordered urgently in an emergency setting — preparation is not the priority
- Inform your doctor about chest pain, shortness of breath, or palpitations you are experiencing
- Inform the doctor about heart or blood pressure medications you take
- A blood sample is drawn from a vein in your arm — results are usually available within 1–2 hours
