What is an ETT?
An ETT (Exercise Tolerance Test), also known as a stress test or treadmill test, evaluates how your heart performs under physical exertion. You walk or run on a treadmill at increasing speeds while your heart rate, blood pressure, and ECG are continuously monitored. Because some heart problems only appear during activity, the ETT can reveal issues that a resting ECG would miss.
Why is an ETT Done?
- To investigate chest pain or shortness of breath that occurs during physical activity
- To detect coronary artery disease — reduced blood flow to the heart often shows up during exercise
- To assess heart function before starting a cardiac rehabilitation or exercise programme
- To evaluate heart rhythm abnormalities that are triggered by exertion
- To determine safe levels of physical activity after a heart attack or heart surgery
- To assess the effectiveness of heart medications or cardiac interventions
What the Results Mean
| Result | Interpretation |
|---|---|
| Normal | Heart rhythm and blood flow remain normal throughout exercise — low risk of significant coronary disease |
| Abnormal (ST depression on ECG) | Suggests reduced blood flow to the heart during exertion — further testing like angiogram may be needed |
| Inconclusive | Result unclear — doctor may recommend imaging-based stress test |
An abnormal ETT does not confirm coronary artery disease on its own — it indicates a higher probability that warrants further investigation. A normal ETT is reassuring but does not completely exclude disease.
How to Prepare for an ETT
- Avoid heavy meals for 2–4 hours before the test
- Avoid caffeine, smoking, and alcohol on the day of the test
- Wear comfortable clothing and supportive sports shoes suitable for walking or running
- Inform your doctor about all medications — some heart and blood pressure medicines may need to be paused as they affect the test result
- Tell your doctor if you have chest pain, dizziness, or breathlessness at rest before the test
- Avoid heavy exercise in the hours leading up to the test
