What is a Coronary Angiogram?
A coronary angiogram is an imaging procedure that examines the blood vessels supplying the heart. A thin flexible tube (catheter) is guided through a blood vessel in the wrist or groin to the heart, and a special contrast dye is injected. X-ray images then reveal whether any coronary arteries are narrowed or blocked. It is considered the gold standard test for diagnosing coronary artery disease.
Why is a Coronary Angiogram Done?
- To investigate chest pain (angina) that has not been explained by other tests like ECG or stress test
- To evaluate the severity of coronary artery disease before deciding on treatment
- To plan coronary angioplasty (stent placement) or bypass surgery
- After a heart attack, to identify which artery was blocked and whether a stent is needed
- To investigate abnormal results on a non-invasive test such as an ETT or stress echocardiogram
What the Results Mean
| Finding | Meaning |
|---|---|
| Normal coronary arteries | No significant blockage — symptoms may have another cause |
| Mild narrowing (below 50%) | Usually managed with medication |
| Significant narrowing (50–70%) | May need stent or medication depending on symptoms |
| Severe narrowing or blockage (above 70%) | Usually requires stent (angioplasty) or bypass surgery |
The cardiologist interprets findings alongside symptoms, heart function, and the number of vessels affected to recommend the best treatment plan.
How to Prepare for a Coronary Angiogram
- Fast for 6–8 hours before the procedure — no food or drink except small sips of water
- Inform your doctor about all medications, especially blood thinners, diabetes medicines, and kidney medicines — some may need to be paused
- Tell your doctor if you have any allergy to contrast dye, iodine, or seafood
- Remove all jewellery, metal objects, and contact lenses before the procedure
- Bring all previous medical records, ECG reports, and echocardiogram results
- Arrange for someone to accompany you — you will need rest and observation after the procedure
