What is the Prostatic Acid Phosphatase Test?

The Prostatic Acid Phosphatase (PAP) test measures the level of acid phosphatase enzyme produced specifically by the prostate gland in men. Before PSA (Prostate Specific Antigen) testing became widely available, PAP was the primary marker used to detect and monitor prostate cancer. While PSA has largely replaced it in routine screening, PAP is still used in specific clinical situations — particularly when PSA results are inconclusive or when monitoring treatment response in advanced prostate cancer.

Why is the Prostatic Acid Phosphatase Test Done?

  • To detect prostate cancer — especially when the disease has spread beyond the prostate gland
  • To monitor treatment response in patients with known prostate cancer on hormonal therapy or chemotherapy
  • To detect cancer recurrence after prostate surgery or radiation therapy
  • To investigate elevated PSA results alongside other prostate markers
  • In forensic medicine — PAP is used to detect seminal fluid in assault cases

Normal Prostatic Acid Phosphatase Levels

ResultPAP Level
Normal (men)0 – 3.0 ng/mL (varies by lab)
Mildly elevated3 – 10 ng/mL — further investigation needed
Significantly elevatedAbove 10 ng/mL — associated with advanced prostate cancer

PAP can also be transiently elevated after prostate massage, rectal examination, or cystoscopy. For this reason, the blood sample should be collected before any prostate examination. Mildly elevated PAP in the absence of symptoms may require repeat testing and PSA comparison rather than immediate action.

How to Prepare for the Prostatic Acid Phosphatase Test

  • The blood sample should be collected before any rectal examination, prostate massage, or cystoscopy — these procedures temporarily raise PAP significantly
  • Avoid bicycle riding or prolonged pressure on the perineal area for 48 hours before the test
  • No fasting is required
  • Inform your doctor about any prostate medications such as finasteride or dutasteride you are taking
  • A simple blood sample is drawn from a vein in your arm
