What is the Total Acid Phosphatase Test?
The Total Acid Phosphatase test measures the combined level of all acid phosphatase enzymes in the blood — produced by multiple tissues including the prostate gland, red blood cells, platelets, bone, liver, and spleen. Unlike the prostatic-specific test, total acid phosphatase is not restricted to prostate tissue. Elevated levels can therefore point to a range of conditions beyond prostate disease, including bone disorders, haematological conditions, and liver disease.
Why is the Total Acid Phosphatase Test Done?
- To screen for prostate cancer alongside PSA — elevated total acid phosphatase with elevated PSA strengthens the suspicion
- To investigate bone diseases — acid phosphatase is elevated in Paget's disease of bone and bone metastases
- To evaluate haematological conditions such as haemolytic anaemia, thrombocytopenia, and Gaucher's disease
- To assess liver disease — acid phosphatase rises in hepatitis and cirrhosis
- To monitor treatment response in prostate cancer alongside PSA and prostatic acid phosphatase

Total acid phosphatase is a non-specific marker — elevation alone requires further investigation to identify the source. Your doctor will order additional tests such as PSA, bone imaging, or liver function tests based on the clinical picture and which tissue source is suspected.
How to Prepare for the Total Acid Phosphatase Test
- Collect the blood sample before any rectal examination or prostate procedure — physical manipulation of the prostate causes a rapid but temporary spike in acid phosphatase
- No fasting is required
- Avoid heavy exercise before the test — muscle activity can contribute to enzyme elevation
- Inform your doctor about all medications you are taking, especially those affecting bone, liver, or the prostate
- A simple blood sample is drawn from a vein in your arm
