What is the Alkaline Phosphatase Test?
The Alkaline Phosphatase (ALP) test measures the level of alkaline phosphatase enzyme in the blood. ALP is found mainly in the liver, bones, kidneys, and intestines. Elevated ALP is one of the most common abnormal findings on a liver function test and can indicate liver disease, bile duct obstruction, or bone disorders. It is routinely included in liver function panels and metabolic panels.
Why is the Alkaline Phosphatase Test Done?
- To investigate liver disease — ALP is particularly elevated in bile duct obstruction, primary biliary cholangitis, and infiltrative liver conditions
- To detect bone disorders such as Paget's disease, bone metastases, or vitamin D deficiency-related bone disease
- To evaluate jaundice — ALP helps distinguish between liver cell damage and bile duct blockage
- As part of a routine liver function panel (LFT) alongside SGPT, SGOT, and bilirubin
- To monitor patients on medications that can affect the liver, such as anti-tuberculosis drugs or statins
Normal Alkaline Phosphatase Levels

Elevated ALP alone does not pinpoint the source — the doctor will check GGT (gamma-GT) to distinguish liver from bone origin. If GGT is also elevated, the problem is likely in the liver or bile ducts. Normal GGT with high ALP suggests a bone source.
How to Prepare for the Alkaline Phosphatase Test
- Fasting for 8–10 hours is recommended — eating, particularly fatty meals, can temporarily raise ALP
- Water is allowed during fasting
- Avoid alcohol before the test
- Inform your doctor about all medications, vitamins, and supplements you are taking — especially anti-TB drugs, statins, or birth control pills
- Avoid heavy exercise before the test if possible
