What is the Anti-Thyroid Antibodies Test?
The Anti-Thyroid Antibodies test checks for abnormal antibodies in the blood that attack the thyroid gland. These antibodies are produced when the immune system mistakenly targets thyroid tissue. The two most commonly tested are Anti-TPO (Anti-Thyroid Peroxidase) and Anti-TG (Anti-Thyroglobulin). This test is the key to diagnosing autoimmune thyroid diseases.
Why is the Anti-Thyroid Antibodies Test Done?
- To diagnose Hashimoto's thyroiditis — the most common cause of hypothyroidism, caused by immune attack on the thyroid
- To diagnose Graves' disease — an autoimmune condition causing hyperthyroidism
- To investigate thyroid nodules or goitre (enlarged thyroid gland)
- To evaluate recurrent miscarriage, as elevated anti-TPO antibodies are linked to pregnancy loss
- To monitor patients with known autoimmune conditions such as type 1 diabetes or rheumatoid arthritis, who have a higher risk of thyroid autoimmunity

Elevated anti-TPO is the most sensitive marker for Hashimoto's thyroiditis. However, mildly elevated levels can also occur in healthy individuals without thyroid disease, so results must always be interpreted alongside TSH, Free T4, and symptoms.
How to Prepare for the Anti-Thyroid Antibodies Test
- No fasting is required
- Inform your doctor about any thyroid medications, steroids, or hormone therapy you are taking
- Stop biotin supplements 24–48 hours before the test if instructed — biotin can interfere with immunoassay-based thyroid tests
- Avoid heavy exercise before the test if possible
- A simple blood sample is collected from a vein in your arm
