What is the LH Test?
The Luteinizing Hormone (LH) test measures the level of LH in the blood. LH is produced by the pituitary gland and works closely with FSH to regulate reproductive function. In women, a sudden surge in LH triggers ovulation — the release of an egg from the ovary. In men, LH stimulates the testes to produce testosterone. It is a key test in the evaluation of fertility and hormonal disorders.
Why is the LH Test Done?
- To investigate infertility in both women and men
- To detect the LH surge and predict the timing of ovulation — important for couples trying to conceive
- To diagnose polycystic ovary syndrome (PCOS) — an elevated LH-to-FSH ratio is a characteristic finding
- To evaluate irregular or absent menstrual periods
- To confirm menopause — LH rises significantly as ovarian function declines
- To investigate low testosterone in men alongside FSH and testosterone tests
- To assess early or delayed puberty in adolescents
Normal LH Levels
| Group | Normal LH Range |
|---|---|
| Women — follicular phase | 2.4 – 12.6 IU/L |
| Women — ovulatory surge (peak) | 14.0 – 95.6 IU/L |
| Women — luteal phase | 1.0 – 11.4 IU/L |
| Post-menopausal women | 7.7 – 58.5 IU/L |
| Adult men | 1.7 – 8.6 IU/L |
In PCOS, the LH-to-FSH ratio is often 2:1 or 3:1 or higher, which is diagnostically significant. LH must always be interpreted alongside FSH, estradiol, and the timing within the menstrual cycle for a meaningful assessment.
How to Prepare for the LH Test
- No fasting is required
- For women, the test is most informative on day 2, 3, or 4 of the menstrual cycle for a baseline reading, unless your doctor requests it at a different time
- Inform your doctor about any hormone therapy, fertility medications, or oral contraceptives you are taking — these directly affect LH levels
- Avoid heavy physical and emotional stress before the test
- A simple blood sample is drawn from a vein in your arm
