This is a routine blood test, so no fasting is needed. Eat and drink as you normally would. Drinking water beforehand helps, since it makes the vein easier to find. Tell the lab about any regular medicines, especially blood thinners like warfarin or aspirin, iron tablets, or steroids, because they can change the reading. If you have had a recent fever, infection, or bleeding, mention it to the person taking your blood.

What is the D-Dimer Test?

The D-Dimer test measures a small protein fragment that is released into the blood when a blood clot dissolves. When a clot forms in the body, the process of breaking it down produces D-dimer. Elevated D-dimer levels suggest that abnormal clotting activity is occurring somewhere in the body. It is most commonly used to rule out dangerous clotting conditions such as deep vein thrombosis (DVT) and pulmonary embolism (PE).

Why is the D-Dimer Test Done?

  • To rule out deep vein thrombosis (DVT) — blood clots in the leg veins causing swelling and pain
  • To rule out pulmonary embolism (PE) — a blood clot in the lungs causing sudden breathlessness and chest pain
  • To evaluate disseminated intravascular coagulation (DIC) — a serious clotting disorder
  • To monitor patients with known clotting disorders or those on anticoagulant therapy
  • During COVID-19 illness, elevated D-dimer indicated severe disease and clotting complications

Normal D-Dimer Levels

ResultD-Dimer LevelInterpretation
NormalBelow 500 ng/mL (0.5 mg/L FEU)Clotting disorder unlikely
ElevatedAbove 500 ng/mLFurther imaging needed to identify cause

An elevated D-dimer does not confirm a clot — it is raised in many other conditions including infection, inflammation, pregnancy, surgery, and cancer. The D-dimer test is most useful as a rule-out test: a normal result makes a significant clot very unlikely. A high result requires follow-up with ultrasound or CT pulmonary angiogram.

How to Prepare for the D-Dimer Test

  • No fasting is required
  • Inform your doctor about blood-thinning medications (anticoagulants) you are taking, as they affect results
  • Inform the doctor about recent surgery, injury, pregnancy, or active infection — all raise D-dimer independently
  • A simple blood sample is drawn from a vein in your arm
  • Results are usually available within a few hours and should be interpreted urgently if a PE or DVT is suspected
