What is the Serum Estradiol (E2) Test?
The serum estradiol test measures the level of estradiol — the most potent and predominant form of estrogen — in the blood. Estradiol is produced mainly by the ovaries in women and in small amounts by the testes in men and by body fat in both sexes. It plays a central role in the development and regulation of the female reproductive system, bone density, and cardiovascular health. It is a key test in fertility investigations and hormonal evaluations.
Why is the Serum Estradiol Test Done?
- To evaluate ovarian function and ovarian reserve alongside FSH and AMH
- To monitor ovarian stimulation during IVF treatment — estradiol rises as follicles grow
- To investigate irregular or absent periods, infertility, or menopausal symptoms
- To diagnose early or delayed puberty in girls
- To evaluate gynecomastia (breast tissue development) in men — elevated estradiol in men can cause this
- To monitor hormone replacement therapy (HRT) in menopausal women
- To investigate osteoporosis in women — low estradiol accelerates bone loss

During IVF stimulation, estradiol levels are monitored closely — each mature follicle is expected to contribute approximately 200–300 pg/mL. Very high levels raise the risk of ovarian hyperstimulation syndrome (OHSS). In a natural cycle, low estradiol alongside high FSH confirms diminished ovarian reserve.
How to Prepare for the Serum Estradiol Test
- No fasting is required
- For baseline assessment, the test is usually done on day 2 or 3 of the menstrual cycle unless your doctor advises otherwise
- Inform your doctor about any hormone therapy, contraceptive pills, or fertility medications you are taking — these significantly alter results
- A simple blood sample is drawn from a vein in your arm
