What is the Serum Progesterone Test?
The serum progesterone test measures the level of progesterone hormone in the blood. Progesterone is produced mainly by the ovaries after ovulation and plays a vital role in preparing the uterus for pregnancy and maintaining it in the early stages. In men and non-pregnant women, small amounts are also produced by the adrenal glands. It is a key test in fertility investigations and pregnancy monitoring.
Why is the Serum Progesterone Test Done?
- To confirm that ovulation has occurred — progesterone rises significantly after ovulation in the mid-luteal phase
- To investigate irregular or absent periods
- To evaluate recurrent miscarriage — low progesterone in early pregnancy may contribute to pregnancy loss

A mid-luteal progesterone above 5 ng/mL generally confirms ovulation occurred. In early pregnancy, a level below 5 ng/mL raises concern for miscarriage or ectopic pregnancy. A single value is less useful than a trend — your doctor may repeat the test over several days in early pregnancy.
How to Prepare for the Serum Progesterone Test
- No fasting is required
- Timing is critical — for confirming ovulation, the test is done on day 21 of a 28-day cycle (7 days after expected ovulation); your doctor will advise the correct day for your cycle length
- Inform your doctor about any progesterone supplements, hormonal contraceptives, or fertility medications you are taking
- A simple blood sample is drawn from a vein in your arm
