What is the Alpha Fetoprotein (AFP) Test?
The Alpha Fetoprotein (AFP) test measures the level of AFP — a protein produced primarily by the fetal liver during pregnancy — in the blood. In adults, AFP is normally present at very low levels. Significantly elevated AFP in a non-pregnant adult can be a sign of certain cancers or liver disease. In pregnancy, AFP levels are measured as part of screening for fetal abnormalities.
Why is the AFP Test Done?
- To screen for and monitor hepatocellular carcinoma (liver cancer) — AFP is the most widely used liver cancer tumour marker
- To monitor treatment response and detect recurrence in patients with liver cancer or testicular cancer
- To detect germ cell tumours of the ovary or testis that produce AFP
- In pregnancy (typically at 15–20 weeks), as part of the maternal serum screening panel for Down syndrome, neural tube defects, and other chromosomal conditions
- To monitor patients with chronic hepatitis B or C and cirrhosis, who are at elevated risk of developing liver cancer

A mildly elevated AFP does not always mean cancer — it can also rise in cirrhosis, hepatitis, or other benign liver conditions. A very high AFP in a patient with liver disease or a testicular mass is a strong indicator of malignancy and requires urgent investigation.
How to Prepare for the AFP Test
- No fasting is required
- Inform your doctor if you are pregnant, as pregnancy naturally raises AFP and changes the interpretation entirely
- Inform the doctor about any ongoing liver disease, chronic hepatitis, or previous cancer history
- A simple blood sample is drawn from a vein in your arm
- Results are usually available within 24 hours
