What is the Anti-CCP Test?
The Anti-CCP (Anti-Cyclic Citrullinated Peptide) test detects antibodies that the immune system produces against citrullinated proteins — proteins modified during inflammation. It is one of the most specific blood tests for rheumatoid arthritis (RA), meaning a positive result strongly points to RA rather than other forms of arthritis. It is particularly valuable because it can become positive years before symptoms of RA even appear.
Why is the Anti-CCP Test Done?
- To diagnose rheumatoid arthritis — especially when the diagnosis is uncertain or symptoms are early
- To differentiate rheumatoid arthritis from other types of joint pain and arthritis
- To predict the severity of rheumatoid arthritis — high anti-CCP levels are associated with more aggressive joint damage
- To evaluate patients with recent-onset joint swelling, morning stiffness, or symmetrical small joint pain
- Often ordered alongside Rheumatoid Factor (RF) — a patient who is positive for both has a high probability of RA

Anti-CCP is approximately 95–98% specific for rheumatoid arthritis — one of the highest specificities of any autoimmune test. However, around 30% of RA patients are anti-CCP negative, so a negative result does not exclude RA. Your rheumatologist will interpret results alongside RF, ESR, CRP, and clinical examination.
How to Prepare for the Anti-CCP Test
- No fasting is required
- No special preparation is needed
- Inform your doctor about any immunosuppressive medications or steroids you are taking, as they may affect the immune response
- A simple blood sample is drawn from a vein in your arm
- Results are usually available within 24–48 hours
