What is the ANA Test?

The Anti-Nuclear Antibody (ANA) test detects antibodies that the immune system produces against the body's own cell nuclei. These autoantibodies are a hallmark of autoimmune diseases — conditions where the immune system mistakenly attacks healthy tissue. ANA is used as a screening test for a range of autoimmune conditions, most notably Systemic Lupus Erythematosus (SLE). A positive ANA alone does not diagnose disease — further specific tests are needed.

Why is the ANA Test Done?

  • To screen for Systemic Lupus Erythematosus (SLE or lupus) — ANA is positive in over 95% of lupus patients
  • To investigate symptoms such as joint pain, skin rashes, unexplained fever, fatigue, or hair loss that suggest an autoimmune cause
  • To evaluate Sjögren's syndrome, mixed connective tissue disease, scleroderma, and other autoimmune conditions
  • To investigate Raynaud's phenomenon — colour changes in fingers triggered by cold
  • As a first-line autoimmune screen alongside CRP, ESR, and CBC

Normal ANA Results

ResultTitreInterpretation
NegativeBelow 1:40Autoimmune disease unlikely
Low positive1:40 – 1:80May be seen in healthy individuals — clinical correlation needed
Positive1:160 or aboveSignificant — further specific antibody testing recommended
Strongly positive1:320 or aboveStrongly associated with autoimmune disease

Up to 20% of healthy individuals can have a low-positive ANA at 1:40. A positive ANA at higher titres, combined with specific symptoms, should prompt further testing for anti-dsDNA, anti-Sm, anti-Ro, and anti-La antibodies to identify the specific autoimmune condition.

How to Prepare for the ANA Test

  • No fasting is required
  • Inform your doctor about steroids or immunosuppressive medications you are taking — they can suppress antibody levels and cause a false negative
  • Inform the doctor about any ongoing infections, as acute infections can temporarily cause a positive ANA
  • A simple blood sample is drawn from a vein in your arm
  • Results are usually available within 24–48 hours
