What is the eGFR (MDRD) Test?
The eGFR (Estimated Glomerular Filtration Rate) is a calculated value — not a standalone blood draw — that estimates how well the kidneys are filtering waste from the blood. It is derived from the serum creatinine result using the MDRD (Modification of Diet in Renal Disease) formula, which accounts for the patient's age, sex, and race. eGFR is the standard measure used to stage chronic kidney disease (CKD) and is automatically calculated by most laboratories when a creatinine test is ordered.
Why is eGFR Done?
- To diagnose and stage chronic kidney disease (CKD) — eGFR determines which of the five CKD stages a patient is in
- To monitor kidney function in patients with diabetes, high blood pressure, or known kidney disease
- To assess kidney function before prescribing medications that are cleared by the kidneys — dose adjustments are often needed when eGFR is low
- To determine whether a patient is suitable for contrast dye in CT scans or MRI
- To evaluate the need for renal replacement therapy (dialysis or transplant) in advanced CKD
eGFR Stages of Kidney Disease

eGFR above 60 with no other signs of kidney damage is generally considered adequate. An eGFR below 60 persisting for more than 3 months confirms CKD regardless of the cause. The MDRD formula can underestimate GFR in healthy individuals — the CKD-EPI formula is increasingly preferred for more accurate estimation.
How to Prepare for the eGFR Test
- eGFR is calculated from serum creatinine — preparation follows the creatinine test
- Avoid eating large amounts of red meat for 24 hours before the test — dietary protein temporarily raises creatinine and affects the eGFR calculation
- Avoid heavy exercise before the test for the same reason
- Stay well hydrated unless your doctor advises fluid restriction
- Inform your doctor about all medications you take, particularly those affecting the kidneys
