What is a Breast MRI?
A breast MRI uses magnetic fields and radio waves to produce highly detailed images of breast tissue. Unlike mammography which uses X-rays, breast MRI involves no radiation and provides a three-dimensional view of the entire breast. It is the most sensitive imaging test available for breast cancer detection, though it is not used as a routine screening tool for everyone โ it is reserved for specific high-risk situations where mammography or ultrasound are insufficient.
Why is a Breast MRI Done?
- Annual screening for women at very high risk of breast cancer โ particularly those with BRCA1 or BRCA2 gene mutations or strong family history
- To evaluate the extent of known breast cancer before surgery โ MRI detects additional tumour sites not seen on mammogram
- To assess the response of breast cancer to neoadjuvant chemotherapy
- To investigate axillary lymph node metastasis when the primary breast tumour cannot be found on other imaging
- To evaluate breast implants for rupture or leakage
- To differentiate scar tissue from tumour recurrence after breast-conserving surgery

How to Prepare for a Breast MRI
- For pre-menopausal women, the scan is ideally scheduled between days 7โ14 of the menstrual cycle โ hormonal breast changes at other times can reduce image quality
- Contrast (gadolinium) is almost always used for breast MRI โ fast for 4 hours beforehand and confirm kidney function with a creatinine test
- Remove all metal objects and inform staff about any implants, pacemakers, or tissue expanders
- You will lie face-down with breasts positioned in dedicated breast coils โ the position is unusual but painless
- Bring previous mammogram and ultrasound reports for the radiologist's comparison
