What is a Mammography?
Mammography is a low-dose X-ray examination of the breast tissue used to detect and diagnose breast abnormalities โ most importantly breast cancer at an early, treatable stage. During the procedure, each breast is briefly compressed between two plates while an X-ray image is taken. A bilateral mammogram examines both breasts in the same session, which allows direct comparison between the two sides โ an important advantage in identifying subtle asymmetries that may indicate disease.
Why is Bilateral Mammography Done?
- Routine breast cancer screening โ particularly recommended for women aged 40 and above, or earlier with a family history of breast cancer
- To investigate a palpable breast lump, nipple discharge, breast pain, or skin changes
- To evaluate breast asymmetry or any abnormality found during clinical examination
- To follow up abnormal findings from a previous mammogram or ultrasound
- Annual surveillance in women with a prior history of breast cancer or high-risk genetic status
- Pre-operative planning before breast surgery
Understanding Mammogram Results (BI-RADS)
| BI-RADS Category | Interpretation | Recommended Action |
|---|---|---|
| 0 โ Incomplete | Additional imaging needed | Return for further views or ultrasound |
| 1 โ Negative | Normal โ no abnormality found | Routine annual screening |
| 2 โ Benign | Definitely benign finding | Routine annual screening |
| 3 โ Probably benign | Less than 2% chance of malignancy | Short-interval follow-up in 6 months |
| 4 โ Suspicious | Biopsy recommended | Tissue biopsy for histology |
| 5 โ Highly suggestive of malignancy | Above 95% chance of cancer | Urgent biopsy and oncology referral |
Mammography detects most but not all breast cancers โ dense breast tissue can obscure abnormalities. Your radiologist may recommend an ultrasound alongside mammography for women with dense breasts.
How to Prepare for a Bilateral Mammogram
- Do not apply deodorant, talcum powder, perfume, or lotion to the breasts or underarms on the day of the test โ these can appear as white spots on the X-ray and mimic calcifications
- Wear a two-piece outfit โ you will need to undress from the waist up
- Schedule the mammogram in the first two weeks of your menstrual cycle when breasts are least tender
- Inform the radiographer if you are pregnant, breastfeeding, or have breast implants
- Bring previous mammogram films or reports for comparison โ this is very important for accurate interpretation
