What is a Cephalometric X-Ray?
A cephalometric X-ray (cephalogram) is a standardised side-profile X-ray of the entire head and face, taken from a fixed distance using a specialised device called a cephalostat. It captures the skull, jaw, teeth, and facial soft tissues in a single image. Unlike a dental X-ray which focuses on individual teeth, a cephalometric view shows the spatial relationships between all the bony and dental structures of the face. It is an essential diagnostic tool in orthodontics and maxillofacial surgery.
Why is a Cephalometric X-Ray Done?
- To plan orthodontic treatment — braces, aligners, or retainers — by assessing jaw and tooth alignment
- To evaluate skeletal relationships between the upper jaw (maxilla) and lower jaw (mandible) before corrective jaw surgery
- To diagnose skeletal malocclusion — underbite, overbite, or crossbite caused by jaw bone discrepancy rather than just tooth position
- To monitor growth and treatment progress in children and adolescents undergoing orthodontic treatment
- To assess upper airway anatomy in patients being evaluated for obstructive sleep apnoea
- Pre-surgical planning for orthognathic (jaw corrective) surgery
Key Measurements in a Cephalometric Analysis

How to Prepare for a Cephalometric X-Ray
- No fasting or special preparation is required
- Remove all jewellery, earrings, hair clips, glasses, and any metal objects from the head and neck before the X-ray
- Inform the radiographer if you are pregnant
- You will be asked to stand or sit still with your teeth in a natural bite position and lips gently closed during the exposure — it takes only seconds
- Bring your orthodontic referral letter and any previous cephalometric X-rays for comparison
