What is the Total Bilirubin Test?
The total bilirubin test measures the combined level of all bilirubin — both direct (conjugated) and indirect (unconjugated) — in the blood. Bilirubin is a yellow waste product formed from the normal breakdown of red blood cells and processed by the liver. When bilirubin accumulates above normal levels, it causes jaundice — the yellowing of skin, eyes, and urine that is one of the most common signs of liver or blood disorders in Bangladesh.
Why is the Total Bilirubin Test Done?
- To evaluate jaundice — the total bilirubin level indicates severity, while direct and indirect fractions identify the cause
- As part of a routine liver function test (LFT) panel
- To monitor newborn jaundice — neonatal hyperbilirubinaemia can cause brain damage if untreated
- To assess liver diseases including hepatitis, cirrhosis, and bile duct obstruction
- To detect haemolytic conditions — excessive red blood cell destruction raises bilirubin
- To monitor patients on medications that affect the liver
Normal Total Bilirubin Levels

Total bilirubin above 1.2 mg/dL is elevated but jaundice only becomes visible in the skin and eyes above approximately 2.5–3.0 mg/dL. Your doctor will order direct and indirect bilirubin fractions alongside the total to determine the underlying cause.
How to Prepare for the Total Bilirubin Test
- Fasting for 4 hours before the test is recommended
- Avoid prolonged exposure to sunlight before the test — light degrades bilirubin and can lower the measured level
- Inform your doctor about all medications you take, particularly those metabolised by the liver
- A simple blood sample is drawn from a vein in your arm
