What is the Serum Calcium Test?
The serum calcium test measures the total level of calcium in your blood. Calcium is the most abundant mineral in the body — it is essential for strong bones and teeth, normal muscle contractions, nerve signalling, and blood clotting. The body tightly regulates calcium levels through the parathyroid glands, vitamin D, and the kidneys. Both high and low calcium can cause significant health problems and require medical attention.
Why is the Serum Calcium Test Done?
- To investigate symptoms of low calcium (hypocalcaemia) — muscle cramps, tingling, numbness, or seizures
- To investigate symptoms of high calcium (hypercalcaemia) — excessive thirst, frequent urination, constipation, fatigue, or kidney stones
- To diagnose hyperparathyroidism — overactive parathyroid glands are the most common cause of high calcium
- To screen for bone disorders, vitamin D deficiency complications, and malignancy-related calcium elevation
- To monitor patients with kidney disease, as the kidneys play a key role in calcium regulation
- Routine part of a metabolic panel or pre-operative workup

How to Prepare for the Serum Calcium Test
- No fasting is strictly required, though some labs recommend it for consistency
- Inform your doctor about calcium supplements, vitamin D supplements, antacids, or diuretics you are taking — all affect results
- Avoid excessive calcium-rich foods before the test if instructed
- A simple blood sample is drawn from a vein in your arm
- The test is often ordered alongside PTH (parathyroid hormone) and vitamin D for a complete picture
