What is the Serum Albumin Test?
The serum albumin test measures the level of albumin โ the most abundant protein in the blood โ produced by the liver. Albumin serves many essential functions: it maintains fluid balance between blood vessels and tissues, transports hormones, drugs, and nutrients around the body, and contributes to the blood's oncotic pressure. Low albumin is a sensitive marker of poor liver function, malnutrition, or chronic illness.
Why is the Serum Albumin Test Done?
- To assess liver function โ the liver is the sole producer of albumin, so low levels indicate reduced synthetic capacity
- To investigate oedema (swelling of legs or abdomen) โ low albumin causes fluid to leak out of blood vessels into tissues
- To evaluate nutritional status in patients who are malnourished, elderly, or recovering from major illness
- To monitor patients with chronic liver disease, cirrhosis, or nephrotic syndrome (kidney protein loss)
- As part of a routine liver function panel (LFT)
- To calculate corrected calcium โ albumin affects the interpretation of total calcium levels
Normal Serum Albumin Levels
| Status | Albumin Level |
|---|---|
| Normal | 3.5 โ 5.0 g/dL |
| Mild hypoalbuminaemia | 3.0 โ 3.5 g/dL |
| Moderate hypoalbuminaemia | 2.5 โ 3.0 g/dL |
| Severe hypoalbuminaemia | Below 2.5 g/dL โ significant clinical concern |
Low albumin is associated with poor outcomes in hospitalised patients and is a marker used to assess disease severity in liver cirrhosis (Child-Pugh score). It can also be low in kidney disease where albumin is lost in urine (nephrotic syndrome), and in severe malnutrition or inflammatory states where production is suppressed.
How to Prepare for the Serum Albumin Test
- No fasting is required for this test
- Inform your doctor about any recent illnesses, surgeries, or significant changes in diet
- Inform the doctor about any diuretics or liver-related medications you are taking
- A simple blood sample is drawn from a vein in your arm
