What is the Serum Ferritin Test?
The serum ferritin test measures the level of ferritin — a protein that stores iron — in your blood. Ferritin is the most sensitive indicator of the body's iron stores. Low ferritin is the earliest sign of iron deficiency, appearing before anaemia develops. High ferritin can indicate iron overload, inflammation, or liver disease. It is often ordered alongside a serum iron and hemoglobin test for a complete picture of iron status.
Why is the Serum Ferritin Test Done?
- To diagnose iron deficiency — the most common nutritional deficiency in Bangladesh, especially in women and children
- To investigate fatigue, hair loss, or poor exercise tolerance that may be caused by low iron stores even without frank anaemia
- To monitor iron supplementation therapy — to confirm stores are being replenished
- To detect iron overload conditions such as haemochromatosis
- To assess iron status before and during pregnancy
- Elevated ferritin is also a marker of inflammation — useful in evaluating conditions like rheumatoid arthritis or liver disease

How to Prepare for the Serum Ferritin Test
- No special preparation or fasting is required
- If possible, have the test done when you are not acutely unwell — ferritin is an acute-phase reactant that rises during any infection or inflammation, which can mask true iron deficiency
- Inform your doctor if you are taking iron supplements, as this affects interpretation
- A simple blood sample is drawn from a vein in your arm
