What is the Serum Iron Test?
The serum iron test measures the amount of iron circulating in your blood at the time of the test. Iron is essential for making hemoglobin — the protein in red blood cells that carries oxygen — and for many other body functions including energy production and immune defence. Serum iron is usually ordered alongside serum ferritin and TIBC (Total Iron Binding Capacity) to get a complete picture of the body's iron status.
Why is the Serum Iron Test Done?
- To investigate anaemia — particularly to determine whether it is caused by iron deficiency
- To diagnose iron deficiency before it progresses to anaemia, especially in women, children, and pregnant patients
- To detect iron overload — conditions like haemochromatosis where excess iron accumulates in organs
- To monitor iron replacement therapy and confirm adequate response to treatment
- To evaluate fatigue, hair loss, or weakness that may be related to poor iron status
- To assess nutritional status in patients with malabsorption conditions

Serum iron fluctuates throughout the day — it is highest in the morning and lower in the evening, which is why a morning fasting sample gives the most reliable result. Low serum iron alongside low ferritin and high TIBC confirms iron deficiency anaemia. High serum iron may suggest iron overload or excessive supplementation.
How to Prepare for the Serum Iron Test
- A morning fasting sample is preferred — fast for 8–12 hours before the test for the most accurate result
- Avoid taking iron supplements for at least 24 hours before the test, as they temporarily raise serum iron
- Inform your doctor about any multivitamins, supplements, or iron injections you have recently received
- A simple blood sample is drawn from a vein in your arm
