What is the Serum Lipase Test?
The serum lipase test measures the level of lipase — an enzyme produced by the pancreas that helps digest fats — in the blood. Under normal circumstances, only small amounts of lipase enter the bloodstream. When the pancreas is inflamed or damaged, lipase leaks into the blood in large quantities. It is the preferred test for diagnosing pancreatitis and is considered more specific than the older amylase test for this purpose.
Why is the Serum Lipase Test Done?
- To diagnose acute pancreatitis — inflammation of the pancreas causing severe upper abdominal pain radiating to the back
- To monitor chronic pancreatitis and assess flare-ups
- To investigate abdominal pain when the cause is unclear
- To evaluate complications of gallstones — gallstones blocking the pancreatic duct are a common cause of pancreatitis
- To detect pancreatic duct obstruction or pancreatic cancer in some cases

In acute pancreatitis, lipase typically rises within 4–8 hours of onset, peaks at 24 hours, and remains elevated for 8–14 days — longer than amylase. A level three times the upper limit of normal is the widely accepted diagnostic threshold for acute pancreatitis, alongside clinical symptoms and imaging.
How to Prepare for the Serum Lipase Test
- Fasting for 8–12 hours is recommended before the test — eating stimulates pancreatic enzyme secretion
- Avoid alcohol before the test, as it can raise lipase levels independently
- Inform your doctor about all medications you take, especially steroids, diuretics, or opioid painkillers, which can affect lipase
- A simple blood sample is drawn from a vein in your arm
