What is the SGOT/AST Test?
The SGOT/AST test measures the level of aspartate aminotransferase (AST), also known as SGOT (Serum Glutamic-Oxaloacetic Transaminase), in the blood. AST is an enzyme found primarily in the liver and heart muscle, with smaller amounts in the kidneys, muscles, and red blood cells. When these tissues are damaged, AST leaks into the bloodstream. It is routinely tested alongside SGPT/ALT as part of a liver function panel.
Why is the SGOT/AST Test Done?
- To evaluate liver health — elevated AST alongside SGPT suggests liver cell damage from hepatitis, fatty liver, or cirrhosis
- To investigate jaundice, abdominal pain, or unexplained fatigue
- To detect heart muscle damage — AST rises after a heart attack, though troponin is now preferred for this purpose
- To monitor patients on medications that can damage the liver, such as anti-TB drugs, statins, or paracetamol in high doses
- To track treatment progress in patients with known liver disease
- As part of a routine liver function panel (LFT) in annual health check-ups

Mild elevation (up to 3 times normal) is common in fatty liver, mild hepatitis, and muscle injury. Levels above 10 times normal strongly suggest acute hepatitis or serious liver damage. The AST-to-ALT ratio also gives diagnostic clues — a ratio above 2:1 often suggests alcoholic liver disease. Results must always be interpreted alongside SGPT, ALP, bilirubin, and clinical findings.
How to Prepare for the SGOT/AST Test
- No strict fasting is required, though many labs recommend fasting for 8 hours for consistency within a liver function panel
- Avoid alcohol for at least 24 hours before the test
- Avoid heavy exercise the day before — intense muscle activity raises AST independently of liver disease
- Inform your doctor about all medications and supplements you take, especially paracetamol, statins, or anti-TB drugs
- A simple blood sample is drawn from a vein in your arm
