What is an MRI of the Ankle?

An MRI of the ankle produces detailed images of the ligaments, tendons, cartilage, and bones of the ankle joint. Ankle injuries are among the most common musculoskeletal problems โ€” but plain X-rays only detect fractures and miss the majority of soft tissue injuries that cause persistent ankle pain and instability. Ankle MRI is the investigation of choice when symptoms persist after an injury or when the cause of chronic ankle pain is unclear.

Why is an Ankle MRI Done?

  • To assess ligament injuries โ€” particularly the anterior talofibular ligament (ATFL), which is the most commonly sprained ankle ligament
  • To evaluate Achilles tendon pathology โ€” tendinopathy, partial tears, or complete rupture
  • To detect osteochondral lesions of the talus โ€” cartilage and bone damage from ankle sprains that causes ongoing pain
  • To investigate peroneal tendon tears or subluxation โ€” a cause of lateral ankle pain and instability
  • To assess posterior tibial tendon dysfunction causing flat foot deformity and inner ankle pain
  • To evaluate stress fractures not visible on plain X-ray

How to Prepare for an Ankle MRI

  • No fasting is required unless contrast is planned
  • Remove all metal objects from the foot and ankle area โ€” anklets, toe rings, and any metallic footwear components
  • Inform staff about any metallic implants, screws, or plates in the ankle or foot from previous surgery
  • Wear or bring loose clothing that allows easy access to the lower leg and ankle
  • The ankle is placed in a dedicated ankle coil at the end of the scanner โ€” only the lower leg enters the machine, which is much less confining than a full-body scan
  • Bring previous X-rays and clinical referral for comparison
