What is an MRI of the Elbow?

An MRI of the elbow produces detailed images of the soft tissues, cartilage, tendons, ligaments, nerves, and bones of the elbow joint. Plain X-rays show bone but miss most soft tissue injuries โ€” which are the most common cause of elbow pain in athletes and working adults. Elbow MRI is the investigation of choice when clinical examination and X-ray are insufficient to explain persistent elbow pain, weakness, or instability.

Why is an Elbow MRI Done?

  • To diagnose lateral epicondylitis (tennis elbow) or medial epicondylitis (golfer's elbow) โ€” tendon degeneration or tears at the elbow
  • To evaluate ligament injuries โ€” particularly the ulnar collateral ligament (UCL) in overhead athletes
  • To assess osteochondral lesions โ€” cartilage and underlying bone damage from repetitive stress
  • To detect loose bodies โ€” fragments of cartilage or bone floating inside the joint causing locking or catching
  • To evaluate ulnar nerve entrapment at the elbow (cubital tunnel syndrome) causing tingling in the ring and little fingers
  • To assess post-traumatic changes after elbow fracture or dislocation

How to Prepare for an Elbow MRI

  • No fasting is required unless contrast is planned โ€” fast for 4 hours if contrast is to be used
  • Remove all metal objects from the arm โ€” watches, rings, and bangles
  • Inform staff about any metallic implants, plates, or screws around the elbow from previous surgery
  • The elbow is typically positioned extended alongside the body or in the "superman" position above the head โ€” the radiographer will advise the best position for the area of interest
  • Bring previous X-rays and any relevant clinical reports
