What is an MRI of the Hip?

An MRI of the hip produces detailed images of the hip joint โ€” including the femoral head, acetabulum, articular cartilage, labrum, surrounding tendons, bursae, and bone marrow. The hip is a deep ball-and-socket joint that is difficult to assess clinically and with plain X-ray alone. Hip MRI detects early cartilage damage, labral tears, and bone marrow changes before they progress to irreversible joint damage, making it invaluable for both younger active patients and older patients with hip pain.

Why is a Hip MRI Done?

  • To detect avascular necrosis (AVN) of the femoral head โ€” death of hip bone tissue due to interrupted blood supply, which MRI detects far earlier than X-ray
  • To evaluate acetabular labral tears โ€” common in young adults with groin pain, often caused by femoroacetabular impingement (FAI)
  • To assess hip joint cartilage damage and early osteoarthritis
  • To investigate stress fractures of the femoral neck โ€” particularly in elderly osteoporotic patients and athletes
  • To evaluate greater trochanteric pain syndrome โ€” gluteal tendon tears or trochanteric bursitis
  • To assess bone marrow oedema from transient osteoporosis of the hip

How to Prepare for a Hip MRI

  • No fasting required for non-contrast MRI โ€” fast 4 hours if contrast or MR arthrogram is planned
  • Remove all metal objects from the waist downwards โ€” belts, keys, coins, and any metal fasteners on clothing
  • Inform staff about any hip replacement prosthesis โ€” metal hip implants cause significant artefact and may make MRI non-diagnostic on that side
  • Wear loose comfortable trousers or change into a hospital gown
  • Bring previous X-rays, bone density scans, and the clinical referral for the radiologist
