What is an MRI of the Upper Abdomen?
An MRI of the upper abdomen produces detailed images of the organs in the upper part of the abdominal cavity โ primarily the liver, gallbladder, bile ducts, pancreas, spleen, kidneys, and adrenal glands. It provides superior soft tissue contrast compared to CT and uses no ionising radiation, making it particularly valuable for characterising liver masses, evaluating the biliary system, and assessing abdominal organs in patients who cannot receive CT contrast dye.
Why is an Upper Abdomen MRI Done?
- To characterise liver lesions detected on ultrasound or CT โ differentiating benign haemangiomas from metastases or hepatocellular carcinoma
- To evaluate the biliary system โ bile duct stones, strictures, or tumours (often combined with MRCP sequences)
- To assess the pancreas โ pancreatitis, pancreatic cysts, or tumours
- To evaluate adrenal masses โ distinguishing benign adenomas from phaeochromocytoma or metastases using chemical shift sequences
- To assess liver fibrosis and cirrhosis with MR elastography in specialised centres
- For patients with contrast allergy or kidney impairment who cannot have CT contrast

How to Prepare for an Upper Abdomen MRI
- Fast for 4โ6 hours before the scan โ an empty stomach reduces bowel motion artefact and allows better visualisation of the pancreas and biliary system
- If contrast is planned, a serum creatinine test is required beforehand
- Remove all metal objects before entering the scan room
- Inform staff about pacemakers, metal implants, or any abdominal surgical clips
- You will be asked to hold your breath briefly several times during the scan โ breathing instructions will be given through the intercom
- Bring previous ultrasound, CT reports, and liver function test results for comparison
