What is a Whole Body MRI?
A whole body MRI is a single comprehensive scan that images from the top of the head to the feet using multiple overlapping stations, creating a complete picture of the entire body without any ionising radiation. It is used for cancer staging, screening high-risk patients for multiple tumours, and detecting disease that has spread to bones, lymph nodes, or organs throughout the body. It is a powerful but time-consuming investigation, typically taking 60โ90 minutes to complete.
Why is a Whole Body MRI Done?
- To stage multiple myeloma โ whole body MRI has replaced whole body X-ray survey as the gold standard for detecting myeloma bone lesions
- To detect bone metastases from cancers such as prostate, breast, lung, or renal cancer โ MRI detects marrow disease earlier than bone scintigraphy
- To screen patients with hereditary cancer syndromes (Li-Fraumeni syndrome, BRCA mutations) for tumours throughout the body
- To evaluate lymphoma staging โ assessing lymph node involvement throughout the body
- To screen for occult primary tumour in patients presenting with metastatic disease of unknown origin
- To monitor treatment response across multiple disease sites in a single radiation-free session

How to Prepare for a Whole Body MRI
- Fast for 4โ6 hours before the scan if contrast is planned
- A serum creatinine test is required beforehand if gadolinium contrast is to be used
- Remove all metal objects from the entire body โ jewellery, piercings, underwire bras, belts, and any metallic clothing items
- Inform staff about all implants throughout the body โ pacemakers, cochlear implants, spinal rods, joint replacements, surgical clips
- Wear comfortable loose clothing without metal fasteners, or change into a hospital gown
- Be prepared for a long examination โ bring earplugs or ask for the provided ones, and expect to lie still for 60โ90 minutes
- Inform staff if you have claustrophobia โ anxiolytic premedication can be arranged
